Long Distance & Local Phone Service Agreement

I agree to pay a monthly recurring charge for telephone service as well as applicable monthly charges for voicemail and 5 cents per minute for long distance service if I have subscribed. In addition, I agree to pay the associated installation charge which is equivalent to one hour of labor at the prevailing hourly rate. I understand that the monthly-recurring charges and the installation charge will be applied to my student account. In addition, I understand that the monthly-recurring charges will be prorated based on when the service was activated.

If I move to another residence hall room, I understand that I must request my line be moved through this web site. I understand that if I do not request that my telephone service be moved, it will remain active in the existing residence hall room and I will continue to be charged for that service. If I fail to have the service moved, I will not be issued credit. I understand that there is a charge to have the service moved which is equivalent to one hour of labor at the prevailing hourly rate. I understand that charges for the move will be applied to my student account.

I understand that if I have long distance service that I am liable for all charges, including those that appear to be fraudulent. I understand that in order to reduce the chances of fraudulent activity that I should keep my authorization code confidential and the privacy of the long distance authorization code is my responsibility. I understand the responsibility for all long distance call charges are mine, if I share my authorization code. If I suspect fraudulent activity, I understand that I should contact Division of IT Tech Support at 573.882.5000.

I understand that my telephone service, including long distance service and voicemail if applicable, will be terminated in May at the close of the spring semester. I understand that if I would like my services to remain active, I must contact Tech Support at 573.882.5000 prior to May 1. I understand that at any time prior to the close of the winter semester, if I no longer intend to live on campus or if I no longer want telephone service in my residence hall room, I must request that the service be canceled through this web site. If I fail to do so, I understand I will not be issued credit. I understand that there is no charge to disconnect the telephone service and that my monthly charges will be prorated depending on the date that I request the service be canceled.

Terms and Conditions

  1. Authorization Code

    1. I understand that in order to reduce the chances of fraudulent activity that I should keep my long distance authorization code confidential, and the privacy of the authorization code is my responsibility.
    2. I understand the responsibility for all long distance call charges are mine, if I share my authorization code.
    3. I understand that if I have long distance service that I am liable for all charges, including those that appear to be fraudulent.
    4. I understand that in order to reduce the chances of fraudulent activity that I should keep my authorization code confidential. If I suspect fraudulent activity, I understand that I should contact Tech Support at 573.882.5000.
  2. Fees and Charges

    1. Long distance service is 5 cents per minute. There are no sign-up or monthly fees, because you are charged only for actual usage.
    2. Charges are applied to your MU student account and monthly detail statements will be sent to your local address. If you do not have a local address on file with the Office of the Registrar, the statements will be sent to your permanent address.
    3. I understand that there is no charge to request or cancel long distance service.
  3. Payment

    1. I agree to pay the long distance charges of 5 cents per minute.
    2. I understand that my long distance service will be terminated in May at the close of the spring semester. I understand that if I would like my services to remain active, I must contact Tech Support at 573.882.5000 prior to May 1.
    3. I understand that my long distance service will be terminated when I am no longer an MU student. I understand that at any time before the conclusion of my student enrollment status, I must cancel long distance service using this long distance form. If I fail to do so, I understand I will not be issued credit.
