Account Code Change Form

Use this form to request MoCode changes to phones, data ports, radios and any other items billed monthly via Source Code CEQ.

The billing cycle runs from the 21st to the 20th of each month and requests received after the 19th will not be changed until the following month. If you have more than 10 billing IDs, e-mail UMC Division of IT Account Code Changes and attach the detail available from PeopleSoft.
